NYC Health + Hospitals/Gouverneur Skilled Nursing Facility Admission Process
Choosing the right type of care for a loved one – along with decisions about the cost of care – can often seem overwhelming. We recognize that the transition of going from a home, assisted living, or a hospital setting into a Skilled Nursing Facility can be a difficult one, both financially and emotionally. Our staff is here to help make this transition as smooth as possible for the resident and family.
We are here to answer your questions and walk you through the admission process, as well as answer any of your questions about insurance and the Medicare and Medicaid programs available to assist residents. You can reach us at 212-441-5018.
We also encourage potential residents and/or family members to schedule a tour of our facility.
