You have a right to request your medical records related to care you received at any of our hospitals under Federal and New York State law. In addition, NYC Health + Hospitals will generally honor a patient’s request to furnish information to another party, which may include but not be limited to a physician, hospital, or medical facility; to an attorney; to court, and to an insurance company, with a written authorization from you or your authorized representative.
