Join the health care team caring for New Yorkers experiencing homelessness
We are seeking energetic and collaborative team members who are passionate about delivering comprehensive primary care to patients experiencing homelessness who have complex social, medical, and behavioral health needs.
There are number of opportunities across our Primary Care Safety Net clinics and the Street Health Outreach and Wellness (SHOW) Street Medicine program. Most positions are designed for care team members to work both in the hospital clinics and in the community. As part of the team, you will receive support and training from NYC Health + Hospitals and your colleagues in other facilities to transform the standard of care for people experiencing homelessness.

Primary Care Safety Net (PCSN)
Primary Care Safety Net practices are enhanced primary care models that serve individuals experiencing homelessness with complex medical, behavioral health and social needs. As a member of a PCSN team, you will be able to:
- Effectively engage people experiencing homelessness with complex barriers to primary care, often through direct outreach on SHOW.
- Provide dignified, trauma-informed care focused on patient-oriented care goals while addressing substance use, mental health, housing progression, and chronic disease.
- Deliver a multidisciplinary care team model in a safety-net health care system combining primary care, social work, care coordination, and nursing.
Services
Safety Net patients receive primary care services, behavioral health care services, and care coordination support from a team of clinical professionals and Community Health Workers, as well as assistance with permanent housing navigation and placement. In our practices, you will be able to provide patients with social and medical complexity with the care they deserve through collaboration with a multidisciplinary team, longer and more frequent visits, and expertise in housing navigation.
Locations
Primary Care Safety Net practices are currently located at Bellevue Hospital in Manhattan, Woodhull Hospital in Brooklyn, Elmhurst Hospital in Queens, and Lincoln Hospital in the Bronx. We are currently accepting applications for all locations.
Street Health Outreach & Wellness
The Street Health Outreach and Wellness (SHOW) program, a fleet of specialized mobile units stationed throughout the city, helps care for New Yorkers who are unsheltered. As a member of the SHOW program, you will be part of an innovative street medicine providing health screenings, vaccinations, wound care, and basic material necessities to unsheltered patients. Additionally, the SHOW team will provide harm-reduction focused addiction services and behavioral health and social service referrals to help meet the needs of this population. In addition to providing care on demand in the field, you will be able to connect patients to ongoing services at NYC Health + Hospitals.
