NYC Health + Hospitals/Sea View is Recognized with Two National Awards for its Palliative Care Program

NYC Health + Hospitals/Sea View, the NYC Health + Hospitals long-term care facility on Staten Island, has received two prestigious national awards for its palliative care program. The awards—one from AMDA: The Foundation for Post-Acute and Long Term Care Medicine, the other from Intalere Healthcare—reflect a continuing tradition of excellence at the 304-bed skilled nursing facility.
NYC Health + Hospitals/Sea View is the only post-acute facility in the nation to receive AMDA’s award, which recognizes the facility’s Enhanced Interdisciplinary Palliative Care Services program. Marian McNamara, RN, associate director of nurses, and Nenita Premian, RN, assistant director of nurses, accepted the award at the AMDA annual conference.
Recognizing that palliative care services were poorly understood by both residents and staff alike, a program was developed to raise awareness and achieve two goals: increase engagement of residents in palliative care and prevent unnecessary hospital readmissions. The Enhanced Interdisciplinary Palliative Care Services program sought to foster an interdisciplinary approach to support both residents and families, enabling the alignment of palliative care with family and residents’ wishes. Over a 14-month period, NYC Health + Hospitals/Sea View doubled the number of actively engaged residents receiving palliative care services. In addition, the facility was able to maintain a very low percentage of patients requiring transfers from NYC Health + Hospitals/Sea View to acute care services (7.8%)–well below the national benchmark (12.9%). As an additional achievement, 98% of residents now have advanced directives in place. The culture of care has changed, too, with staff now embracing palliative care, as well as curative care.
NYC Health + Hospitals/Sea View is one of only three facilities in the nation to receive Intalere Healthcare’s Achievement Award in the “Quality/Patient Care Delivery and/or Patient Satisfaction” category. At an event held between May 15 and 18, NYC Health + Hospitals/Sea View’s palliative program will be recognized for having created “incremental value and greater operational efficiencies through constant innovation and transfer of best practices.” NYC Health + Hospitals/Sea View is the sole recipient of this award in the Greater New York City area.

Earlier, NYC Health + Hospitals/Sea View was named one of the “Best Nursing Homes” nationally for 2016-2017 by U.S. News & World Report, and for nine years running has maintained a consistent five-star rating from the national Centers for Medicare and Medicaid Services. It consistently ranks nationally in the top 5% of all long-term care facilities.
NYC Health + Hospitals/Sea View is situated on a 70-acre campus in the Staten Island Greenbelt.
