NYC Health + Hospitals Earns 2026 Leapfrog “Full Transparency” Badge Recognition Across All Hospitals
Hospitals completed and submitted all sections of the 2026 Leapfrog Hospital Survey, demonstrating a commitment to transparency, patient safety, and quality improvement

NYC Health + Hospitals today announced that all 11 of its hospitals have earned the new Leapfrog “Full Transparency” badge for completing, affirming, and submitting all sections of the 2026 Leapfrog Hospital Survey. The recognition underscores NYC Health + Hospitals’ commitment to transparency and to making important information about patient safety and quality available to the public.

The Leapfrog Group, an independent national watchdog organization that assesses hospital and ambulatory surgery center performance, introduced the Full Transparency badge to recognize hospitals that complete, affirm, and submit all sections of the 2026 Leapfrog Hospital Survey.
According to Leapfrog, transparency is a powerful driver of change in health care, helping hospitals identify opportunities for improvement while giving patients access to information about quality and safety.
The 2026 Leapfrog Hospital Survey includes nine sections covering key areas of hospital quality and patient safety:
- Section 1: Patient Rights and Ethics
- Section 2: Medication Safety
- Section 3: Adult and Pediatric Complex Surgery
- Section 4: Maternity Care
- Section 5: Physician and Nurse Staffing
- Section 6: Patient Safety Practices
- Section 7: Managing Serious Errors, including infection data from the National Healthcare Safety Network (NHSN)
- Section 8: Pediatric Care
- Section 9: Outpatient Procedures
Completing the full survey requires hospitals to provide information across a broad range of clinical, operational, and patient safety measures. The data contributes to the information available to patients and families through Leapfrog’s public ratings platform and supports hospitals in identifying areas where continued improvement can make care safer and more effective.
