A Patient-Centered Medical Home (PCMH) is not a physical place for you to get care – it is way of giving care that is about a partnership between you, your personal doctor, and your care team. At a PCMH practice, you build a trusting, long-term relationship with your doctor and your care team, and you work together to make sure that you get the care you need to stay healthy.
At a PCMH practice, you work with a dedicated care team (your doctor and other staff who work at the clinic to keep you healthy) to create a personalized care plan to help you meet your health goals. Whether you need extra help with managing a chronic condition like diabetes, want to lose weight, or want to quit smoking, you can depend on the support of your care team.
Your PCMH care team can help you:
Understand and manage your medications
Arrange for any tests or treatments that you may need
Coordinate your visits to specialists
Find useful resources, like support groups or health classes
Get answers to your health questions when you need them. Many of our providers have evening and weekend hours, offer same-day appointments, and are available to answer your questions by phone or secure message.
