The Partnership In Care Program – a special, enhanced care coordination plan offered for HIV+ patients by MetroPlus. Enrollees benefit from a host of supports, enhanced access, and perks such as:
- Enhanced scheduling – you can see multiple providers during a single visit
- peer counselor/care facilitators
- Member engagement and enrichment workshops
- Auto reminders to assist with prescription refills and scheduling follow up care
- Wellness Rewards (shopping rewards earned for making healthy choices)
