Community Health Education and Outreach

NYC Health + Hospitals/Lincoln is committed to improving the health status of the Bronx community. Our dedicated team of fully credentialed public health educators promote healthy living by raising awareness about accessible preventive and wellness programs, which Lincoln offers at little or no cost to the communities we serve.

Lincoln’s Community Health Education and Outreach program is nationally recognized for award winning preventive health and screening programs. Our team of highly skilled educators collaborates with our doctors and nurses to offer a dynamic curriculum of innovative healthcare programs to community groups. The team conducts workshops, health fairs, health screenings, and educational forums on a variety of health topics, including diabetes, high blood pressure, asthma, cancer, behavioral health, women’s and men’s health, senior health, health insurance, nutrition and many others.

Community Recognitions

2016

  • On September 24th, the NAACP/Parkchester Branch presented Lincoln’s Outreach team with the “Health Champions’ medal during their 9th annual health and wellness fair celebrating exemplary work performed in servicing Bronx seniors, families, children, adolescents and community residents.
  • Ruben Diaz Jr, President of the Borough of the Bronx, presents a Citation of Merit-in recognition of the Outreach team’s commitment and dedication to reaching out to the Bronx community on August 13.
  • The Church of the Revelation presents the team with the ‘Victory Bronze Torch’ medal for outstanding community and partnership award on August 13 during their 6th annual health and wellness fair.

2015

  • Weill Cornell Medicine Medical College Alumni Association awarded Lincoln’s outreach team with an appreciation award for Community Perspectives in Medicine.
