The Macarther Initiative on Depression & Primary Care at Dartmouth & Duke: Depression Management Tool Kit
This tool kit was developed to aid primary care clinicians in caring for their patients who suffer from depression. The case management process recommended in this tool kit builds on the Agency for Healthcare Research and Quality and other evidence-based sources.

Patient Health Questionnaire (PHQ-9) Screening Tool
PHQ-9 may be used when a clinician suspects depression and/or the two-question screen is positive. The PHQ-9 can be self-administered before, during, or after the office visit (The Macarther Initiative on Depression & Primary Care at Dartmouth & Duke, 2004).
